Enclosed are the following forms to read, fill out, and return to Pre-school office on or before Open House Aug 20th, 2018, so that the proper preschool registration meets Department of Children & Families Services and 4C Guidelines.

2. Know your Child Care Facility
3. Permission for Medical Care 
4. Permission to Photograph or Tape 
5. SCPS Release & Consent
